Providence Community Health Napa Valley, founded in 1979 as Hospice of Napa Valley, is a leading provider of compassionate care for those facing the challenges of aging, Alzheimer’s disease and other serious illness or the end of life. Formerly known as Collabria Care we are now part of the Providence health system, and our services include:
- Providence Adult Health of Napa Valley
- Providence PACE
- Providence Palliative Care of Napa Valley
- Providence Hospice of Napa Valley
For adults with dementia, developmental disabilities, and other serious illness, our Adult Day Health program helps participants stay mentally and physically active and offers personalized nursing services and therapeutic activities. As the Napa County Alzheimer’s Day Care Resource Center, we provide crucial caregiver respite, support groups, and community education.
Through PACE (Program of All-Inclusive Care for the Elderly) we provide comprehensive medical care and social services to qualifying older adults. PACE helps them remain independent, living at home for as long as possible.
With a focus on comfort and quality of life, our Palliative Care and Hospice teams provide compassionate medical care for those who are facing the end of life and grief support for those living with loss.
